Is this the Amendment 4 form?

Yes. It follows the IET's A4 model Minor Works certificate sections (description, earthing and bonding, circuit details, test results, declaration), including the split TN-C-S (PME) and TN-C-S (PNB) earthing options and the r1-r1, rn-rn, r2-r2 ring readings. Check it against your scheme's requirements.

Can I use A3 forms after 15 October 2026?

BS 7671:2018+A2:2022+A3:2024 is withdrawn on 15 October 2026. Work designed after that date should be certified to Amendment 4.

Will it stop me issuing a failing certificate?

It warns you if polarity isn't satisfactory, insulation resistance is under 1 MΩ or the RCD takes longer than 300 ms, because a minor works certificate should only be issued for work that complies.
